MTM on demand is an online information platform for Medication Therapy Management (MTM) for patients and providers. 

Knowing more about one’s health helps to ensure that patients achieve the best outcomes with their medications (prescribed, over the counter and herbs). 

Patients

MTM, Medication Therapy Management is a range of services provided by pharmacists to help  identify drug therapy problems .

Anyone who uses prescription and non-prescription medication may benefit from an MTM service.  

During an MTM visit a pharmacist will provide

  • Medication therapy review (MTR)
  • Personal medication record (PMR)
  • Medication-related action plan (MAP)
  • Intervention and/or referral
  • Documentation and follow-up

The goal of the MTM service is to better understand your medications and take charge of your health. 

Providers

Optimize patient therapy, become a provider and improve patient outcomes.

MTM and
Preventative Health

During your MTM visit the focus is to improve your health with your current medication therapy including over the counter medications in addition to preventative health services that keep you healthy.

MTM and Vaccines

Most vaccines are covered by insurance at no cost to the patient. At an MTM visit you may meet with a pharmacist that can administer needed vaccines or provide referral regarding vaccines that you are missing. 

Gene testing
and your medications

Some MTM programs may use pharmacogenomic test results which help make certain medications personalized to the patient’s DNA making the response to therapy even more effective.

More studies are currently being conducted to evaluate genetics and response to certain medications.
